What this test measures
- PhosphorusFrom the lab
A mineral that pairs with calcium in bone and forms part of the body's energy currency.
A phosphorus blood test measures the inorganic phosphate in your blood; depending on the laboratory, the result is labelled phosphorus or phosphate. Phosphate pairs with calcium in bone and is part of how cells make and use energy. Its level is set by the kidneys, the gut, vitamin D and parathyroid hormone, so the result is usually read alongside calcium and kidney function.
What it measures
Most of the body's phosphate is stored in bone, combined with calcium. The rest helps balance fluids and acidity, supports energy production, and helps nerves and muscles work. It comes from foods such as dairy, meat, poultry, eggs, nuts, seeds and dried beans.
The kidneys remove extra phosphate in urine and hold on to it when levels are low, and the gut controls how much is absorbed from food. Vitamin D and parathyroid hormone (PTH) both act on it, and phosphate tends to move in the opposite direction to calcium. In the laboratory, inorganic phosphate is measured through a chemical reaction with molybdate.
Normal range
| Result | mmol/L | mg/dL |
|---|---|---|
| Typical adult range | 0.81 to 1.45 | 2.5 to 4.5 |
| Children | higher than adults | 4.0 to 7.0 |
| Low phosphate (hypophosphataemia), adults | below 0.8 | below about 2.5 |
| Severe low phosphate | below 0.32 | below about 1.0 |
Ranges vary by laboratory, method, age and sex. MedlinePlus, for example, uses 2.8 rather than 2.5 mg/dL as the adult lower limit, so read your result against the range on your own report. Children run higher because their bones are still growing. A mildly low result is often not a health problem on its own.
Time of day and meals
Phosphate follows a daily rhythm. In a study of six healthy men sampled every hour for 24 hours, serum phosphorus fell through the early morning to a low point around 11:00, rose to a plateau by about 16:00 and peaked around 00:30. Diet shifts it too. Ten days of a low-phosphorus diet lowered the 24-hour average by about 40%, while extra phosphorus raised the 24-hour average by about 14% without changing the morning fasting level. That is why a morning sample, taken fasting, makes one test easier to compare with the next.
What a high phosphorus usually points to
- Advanced chronic kidney disease or kidney failure, as the kidneys clear less phosphate.
- Underactive parathyroid glands (hypoparathyroidism).
- Too much vitamin D, or a great deal of phosphate in the diet.
- Phosphate-containing laxatives or enemas, and long-term steroid use.
- Acidosis, including diabetic ketoacidosis.
- A falsely high reading, from haemolysis in the sample, or from interference by abnormal blood proteins (a gammopathy), very high blood fats, high bilirubin, or liposomal amphotericin, an antifungal medicine.
Next, a doctor typically reads phosphorus with kidney function (creatinine and eGFR) and calcium, and may add PTH and vitamin D, which are often ordered together with phosphate. Because laboratory interference is one of the explanations, an unexpected high reading on an otherwise normal panel may simply be repeated.
What a low phosphorus usually points to
- Overactive parathyroid glands (hyperparathyroidism).
- Low vitamin D, which over time can soften bone (osteomalacia in adults, rickets in children).
- Refeeding after a period of very poor nutrition, the acute cause most often cited in the guidelines reviewed in a 2025 umbrella review.
- Heavy alcohol use and very poor nutrition.
- Some iron infusions, notably ferric carboxymaltose, especially when repeated.
- Long-term use of some antacids (aluminium hydroxide or calcium carbonate) and certain diuretics.
What a doctor typically looks at next is calcium, PTH and vitamin D, sometimes a urine phosphate test, and when the sample was taken in relation to meals.
What moves phosphorus
- Time of day and recent meals, as above.
- Age. Children's levels are higher.
- Haemolysis, which raises the measured value.
- Medicines. Phosphate laxatives and enemas raise it; some antacids, diuretics and iron infusions lower it; large amounts of intravenous mannitol can make it read falsely low.
Preparing for the test
You may be asked to fast for several hours before a phosphorus test, and a morning sample makes results easier to compare over time. Tell the person taking the sample about medicines, supplements and over-the-counter products, especially antacids, laxatives, enemas and vitamin D. Whether to pause any of them is a question for your doctor.
How Clare's panel handles it
In Clare's panel, phosphorus is measured by the lab, alongside calcium, magnesium, 25-OH vitamin D, alkaline phosphatase (ALP) and creatinine, with eGFR calculated from creatinine, so the results it is usually read with are on one report. When Clare opens, results will sit in one health account with your earlier ones, with plain explanations in English and Arabic.
Why it matters in the UAE
Low vitamin D is common here: in a study of 392 Emirati adults screened in Abu Dhabi, 79.6% had 25-OH vitamin D below 50 nmol/L. Vitamin D affects phosphorus as well as calcium. The Endocrine Society notes that without vitamin D the gut absorbs about 60% of dietary phosphorus, and that the parathyroid response which follows makes the kidneys lose more phosphorus, so blood phosphorus often runs low-normal or low. Reading phosphorus beside 25-OH vitamin D helps make sense of both. Vitamin D in the UAE: a sunny country, running low covers why levels run low in the UAE.
Price in the UAE
Phosphorus is not usually sold on its own; it usually comes within a kidney function test, though not every package lists it. When we read public prices on 11 October 2026, a kidney function test cost from 89 to 900 at 6 UAE providers. VAT is rarely stated, some are offer prices or starting prices, and home collection can add a fee. Check that the package lists phosphorus or phosphate before buying.
Related tests
Phosphorus is read with calcium, its partner in bone, with 25-OH vitamin D, and with creatinine and eGFR, which show how well the kidneys are clearing it. Alkaline phosphatase (ALP), an enzyme that comes partly from bone, and magnesium complete the mineral picture.
Why the number means more over time
Phosphorus moves through the day and with meals, so comparisons are fairest when samples are taken the same way each time, in the morning and fasting. Across a series, a level that climbs as eGFR falls tells a different story from a single high reading on a haemolysed sample. If your phosphorus sits outside the range or is changing across tests, talk to a doctor about it with the results in front of you. Clare is opening with a waitlist at clare.health.
